REGISTER FOR WAITING LIST MOBILE PASSPORT SERVICE 2018

Warning:
1. Please do not register more than once for the same person. If you do so, we will keep the last register that you registered with us ONLY. This may affect your place in the quota.
2. Please upload all required documents on Section B or Section C.
3. Your appointment time and date will be allocated for you. We will contact you by phone after Monday 26th of March 2018 onward to finalise your confirmation before we arrange your appointment time and date.

Our quota has now been reached. Any further registrations will be placed on a Waiting List.

Registrations must be received between Thursday 1st of February 2018 from 9.30AM (WA Time) to Wednesday 28th of February 2018 before 2.30PM (WA Time). Registrations will NOT be accepted before or after this period.

Priority for renewing Thai passport will be given to the following categories:
1. Lost and stolen passport: If you have the Thai Embassy Report of Loss or Theft of Thai Passport form is stamped and signed by police within 6 months of the issued date).
2. Expired passport
3. Any changes in your personal information e.g. changed your Title/First name/Last name
4. Passport expiry date is prior to 1 st April 2019
Note: If you registered for mobile passport service in Perth 2018 and your passport expiry date is later than 29 th April 2019 and not in any above categories (1-4), you will be placed on the WAITING LIST.

Note: 1. To renew a Thai Passport in Perth during the Mobile Passport Renewal service, you must have either a valid Thai Identification Card or a valid Thai Passport.
   2. If this is your first Thai passport application, you must have a Thai Birth Certificate.
   3. If the child is under 20 years old,
    3.1. Both of applicant’s parents must attend to the appointment with the applicant. or
    3.2. One parent who is unable to attend the appointment must provide an original Letter of Consent (3 months validity from the date it is issued on the letter) with another parent and applicant on the appointment date. or
    3.3. One parent is deceased. The original and a photocopy of Death Certificate (all pages) must be provided with other parent and applicant on the appointment date. or
    3.4. The parents are divorced. The original and a photocopy of Divorce Certificate (all pages must include an amendment awarding custody of child to the parent concerned) or Letter of Custody/Guardianship (Por Kor 14) from the Thai government awarding custody to the parent concerned must be provided with Custody parent and applicant on the appointment date. Otherwise go to section 3.2. or
    3.5. One or both parents are missing or have been hospitalised. The Original and a photocopy of Court Order (proving custody or guardianship) must be provided with Custody parent or or guardianship and applicant on the appointment date. or
    3.6. The parents have adopted a Thai child. The Original and a photocopy of Proof of Adoption (all documentation) must be provided with parents and applicant on the appointment date.
